By scheduling with Soul oasis, you are agreeing to the following policies:

Payment

Full payment is expected at the time of service. Types of payments: Cash, credit/debit card (Fees applies), Venmo, cash-app. checks are not accepted. We require a valid credit card to hold on file to hold all appointments. 

Scheduling/Appointment Time:

In spa, first time appointment, please arrive 15 minutes prior to your appointment time to allow time to complete the client intake form. For all other appointments, please arrive 5 minutes time before your appointment. 

Mobile massage: expect arrival 15-20 min prior to your appointment time.  this time will allow set up and get ready for massage.  Please have a 10 x 8 space to accommodate the massage table. Any furniture should be moved out the way before arriving.

CANCELLATION & NO-SHOW POLICY:

Time is valuable. Your appointment is specifically for you, please be courteous and respect ours. We require at least 24 hr. notice to cancel an appointment. clients who Cancel LESS than 24hrs notice will be billed 50% of the price of scheduled services. No call, No show will be billed full price of the service scheduled. 

Informed consent and Confidentiality:

prior to each massage session, the treatment plan will be discussed with you.

when booking your first appointment, you will receive a link with a medical intake form. Please fill this form and sign it the day before your appointment. Massage therapy is a form of medical wellness. Your medical history is important and can reveal whether or not massage is indicated for you or if modifications need to be made. 

Clients will sign and receive an updated form each year. Failure to complete the require documentation will result in cancellation of your appointment. Please answer the questions truthfully. Your information will not be discussed without a signed medical release document.

Medical History:

It is the responsibility of the client to keep the therapist informed of any medical treatment or prescriptions medications taken. in some cases, you may be asked to provide a written release from your physician in order to receive massage therapy. 

MASSAGE TERMINATION:

Only professional massage and bodywork services for relaxation or therapeutic purposes are offered at this massage practice. Massage services will be terminated immediately in the event of inappropriate conduct of any kind. This includes harassment, threating speech or behavior, sexual advances or request, or disrespectful actions or languages. A session will not be conducted if the client is under the influence of drugs or alcohol. If massage is terminated for any reasons, full payment for scheduled session is still required and no future appointments will be granted.

Draping policy:

Clients will be appropriately draped with a sheet and/or towel at all times. only areas of the body that are currently being treated will be exposed. The Breast and genital areas will always remain draped and are never massaged.

Right of Refusal:

We have the right to refuse clients for any reason. You have the right to refuse services for any reason

Minors: 

Children under 18yr are not allowed in spa unless they are receiving services. If a minor is receiving services, parents will have to be in the room with them while they are receiving massage. 

Personal Hygiene: It is recommended to shower before and after massage. Showers are available at the spa. shower is located down the hall, take a left at the back waiting area, the door to your right is the shower. showers 1 guest at a time.
